Release & Consent Form.

fax (801-581-4091), or in person.

Fall & Spring Payment Option
Families may elect to prepay CDT tuition for the entire year (fall and spring sessions). Families who choose this option will receive a 5%
discount. Fall/Spring tuition for CDT 2nd and 3rd grade will be $595.00. Fall/Spring tuition for CDT 4th grade and up will be $823.00. Families who do not choose
this option will be billed in November for Spring session tuition. Spring tuition will be the same as fall tuition.
